If you want to go with your team to State Playoffs in the State of Tennessee you will need to submitt a request for a formal TSSAA medical credential card. Requests for a credential must be completed annually.

The TSSAA will be distributing such passes again for the 2011-2012 season. Cards will only be issued twice during the year: September and January. Please submit any such requests for a card prior to September 1st to receive your card during the September 2011 distribution.

As a general reminder, please be sure to list your Tennessee License number on your application, not a professional certification number. Also, you must list the name of the specific school with which you have employment or a formal relationship. Generic cards are not issued.
